There are regular updates/What’s New for Alexa/EchoDot. Are these installed automatically or do they need to be installed manually?  If manually, where do I find the updates?

If you have set your app to update automatically, it will. Otherwise do that manually.
The echos themselves will update automatically. The website of course always is the latest version.

How do we know when our Echo has been updated and where do we find out what features have been added?  It would be interesting to know when Amazon fixes a bug or adds a feature.

Go to
https://alexa.amazon.com/spa/index.html#settings
Select your device then scroll down to About > Device software version
Example : my Echo 1st gen reads 599469720

Then go to
https://www.amazon.co.uk/gp/help/customer/display.html?nodeId=201602210
to check if you're up to date.
