Echo talks first

Yesterday Alexa just started talking on her own.  The first time she said something about getting connected although she was connected at the time.  The second time she gave me a weather report without being asked.  The TV was on in the room but I don't recall anyone saying "Alexa" or "weather".

Could the unit be defective?  I've only had it for a couple of weeks and I don't want to miss being able to return it for a replacement.

You did not hear a wake word but Alexa did.  Several people have reported that the Echo will sometime respond when the tv is on.  I would not worry about it just yet.

it could be a combination of words that may have sounded like alexa.  In addition, saying lexa and other similar sounding words or names could wake up your echo.  Finally sound gets garbled the further it goes so just about anything could work if misunderstood.
